*I understand the inherent risks of getting a tattoo, including infection, scarring, difficulty detecting melanoma, and allergic reactions to pigments or materials. Aware of these risks, I choose to proceed with the tattoo application and accept all associated risks.

*I currently do not have any symptoms suggesting any communicable illness.

*I am not under the influence of alcohol or drugs and am willingly choosing to be tattooed by Artist.

*I have eaten within the past few hours

*The Artist and Tattoo Studio are not responsible for the meaning or spelling of the symbol or text I chose.

*I acknowledge that a tattoo permanently alters my appearance and can only be removed by laser surgery.

*The Artist and Velvet Tattoo Parlor LLC have given me tattoo care instructions, which I understand and will follow. I acknowledge that not following these instructions may lead to infection, and if touch-ups are needed due to my negligence, I will pay for them.

*I understand that there may be color and design variations between the selected art and the tattoo. Over time, the colors and clarity will fade due to pigment dispersion under the skin.

*I release all to photographs of me and my tattoo, consenting to their reproduction. If you disagree, please inform your Artist.

*I understand that all procedures are non-refundable.

*I agree to reimburse the Artist and Tattoo Studio for any attorneys' fees and costs incurred in any legal action I bring against them, where they are the prevailing party. I accept that the courts of New York, USA, will have personal jurisdiction and exclusive venue any disputes related to this agreement.

*I acknowledge that I have had enough time to read this document and Article D (included on https://www.velvettattooparlor.com/article-d), that my questions have been answered, and that I am signing a legal waiver of certain rights against the Artist and the Tattoo. If any part of this release is found invalid, it will be removed, and the rest will be interpreted as if it was never included.
